HAB45 is a grant-funded project steered by our Executive Committee and our Leadership Council. We are headquartered at CocoKids, which is our fiscal agent.
Executive Committee
HAB45 is governed by a strong Executive Committee made up of leadership and decision-makers from key healthcare and community organizations serving families with young children in Contra Costa County, California. The Executive Committee provides oversight and sets the overall strategic direction of the collaborative. We also work closely with our longtime advocacy partners Rising Juntos to center the experiences and priorities of low-income families in Contra Costa.

Leadership Council
Healthy & Active Before 5’s Leadership Council is made up of organizations serving young children and their families in Contra Costa County. Members meet biannually to learn about issues affecting children’s health and health equity, build relationships across sectors, and participate in advocacy.
The Leadership Council acts as our community sounding board. In 2007, our Leadership Council provided invaluable guidance and consensus in the development of our original Action Plan. Now, we ask the Leadership Council to help with implementing our Public Policy Agenda and best practices for supporting children’s health within the wider community.
